GN 01742.205 Preparing Claims Packages in OIO for Claims Filed Outside Korea

 

A. Procedure — Initial claims packages

Prepare initial claims packages to request Korean coverage records and to inform the NPC about claims for Korean benefits. (See GN 01741.205 – GN 01741.220 for the applications and evidence required to prepare the packages.)

B. Procedure — U.S. claims

Prepare form SSA-2960-KOR to request the Korean coverage record if the Korean record is not in the claims file.

C. Procedure — U.S. and Korean or Korean claim only

Prepare a claims package that includes:

  • •

    Form SSA-2960-KOR

  • •

    The special Korean application KOR-USA 1

  • •

    U.S. totalization earnings record

  • •

    Medical evidence from SSA's files, if the Korean claim is for disability benefits.

    Note: SSA files include claims folders in OIO, in OCO's Megasite or in the caves in Kansas City.

  • •

    Evidence submitted for the Korean claim, and

  • •

    A request for the Korean coverage record, if one is needed
