GN 01746.106 How to Evaluate Japanese Coverage Certifications

A. Introduction

There are some entries that must be shown on the Japanese coverage record before it can be used to credit Japanese coverage. GN 01746.106B describes the entries required for an acceptable Japanese coverage record.

B. List of Required Entries

Below is a list of required entries:

  • The correct worker's name must be shown in Item 1 of Form J/USA 5.

  • Entries in the “Type of period of coverage”, “Date of enrollment”, “Date of withdrawal”, and “Number of months covered” columns should be present for each period of coverage listed in Item 2.

  • The form J/USA 5 must show the date of certification and signature of the Japanese official verifying the information.

  • If any of the required information is missing, route the case to Division of Training and Program Support (DTPS) for evaluation.
