GN 01746.205 Preparing Claims Packages in DIO for Claims Filed Outside Japan

A. Procedure - Initial Claims Packages

Prepare initial claims packages to request Japanese coverage records and to inform the Japanese agency about claims for Japanese benefits. (See GN 01745.205GN 01745.220 for the applications and evidence required to prepare the packages.)

B. Procedure - U.S. Claims

Prepare form SSA-e2960-J and forward it to the appropriate Japanese agency with a copy of form J/USA 4 to request the Japanese coverage record if the Japanese record is not in the claims file.

C. Procedure - U.S. and Japanese or Japanese Claim Only

Prepare a claims package that includes:

  • Form SSA-e2960-J to certify the filing date for Japanese benefits, established factors of entitlement, and U.S. benefit information

  • The special Japanese application form (either J/USA 1 or J/USA 2)

  • A U.S. totalization earnings record

  • Medical evidence from SSA's files, if the Japanese claim is for disability benefits.

    NOTE: SSA files include claims folders in DIO, in the caves in Kansas City.

  • Evidence submitted for the Japanese claim, and

  • A request for the Japanese coverage record, including form J/USA 4, if one is needed
