GN 01746.210 Preparing Claims Packages in DIO for Claims Filed in Japan

A. Procedure - U.S. Claim

Prepare an SSA-e2960-J if a Japanese coverage record is required and has not been requested. Include a copy of form J/USA 4 with the request.

B. Procedure - U.S. and Japanese or Japanese Claim Only

Prepare an SSA-e2960-J to:

  • •

    Request the Japanese coverage record if one has not been requested, include a copy of form J/USA 4, or

  • •

    Send the Japanese Agency the information or documentation they have requested.

    NOTE: Under this agreement, most correspondence on liaison issues should take place between the Japanese agencies and FSP-Manila.
