GN 01748.205 Preparing Claims Packages in DIO for Claims Filed Outside Denmark

A. Initial claims packages

Prepare initial claims packages to request Danish coverage records and to inform the Danish agency about claims for Danish benefits.

B. U.S. claims

Prepare Form SSA-2960 (USA/DK3) and forward it to the Danish agency if the Danish record is not in the claims file.

C. U.S. claims/Danish totalization benefits

The Division of International Operations (DIO) takes the following action.

Sends the Danish agency the following:

  • A liaison form to request a Danish Coverage record to certify the date for the Danish benefits and to certify established factors of entitlement, such as date of birth.

  • A copy of SSA-2490 (Part 1), SSA -2960 and ICERS record used to apply Danish Totalization benefits.

  • Available medical evidence if the claim is for disability benefits.

D. Procedure - Claim for Denmark benefits only (U.S. resident)

DIO takes the following action.

Sends the Danish agency the following:

  • A liaison form to certify the filing date for Danish benefits and to certify established factors of entitlement such as date of birth

  • Part I and II of the SSA-2490 used to apply for Danish benefits (Totalization or non totalization)

  • A U.S. earnings record

  • Available supporting evidence including medical evidence, if the claim is for disability benefits.

DIO will send a letter to the claimant advising them that his/her claim was forwarded to the Danish Agency.
