A. Procedure – U.S. claim

Prepare a SSA-2960 (USA/DK3) if a Danish coverage record is required and has not been requested.

B. Procedure – U.S. and Danish or Danish claim only

Prepare an SSA-2960 (USA/DK3) to:

    Request the Danish coverage record if one has not been requested, include a copy of the SSA- 2960 (USA/DK3).

  • •

    Send the Danish Agency the information or documentation they have requested.
