GN 01752.012 How the Foreign Service Post in Warsaw, Poland Processes and Develops Claims Filed With the ZUS or KRUS Agency

A. Process – person is not eligible for Polish benefits

When a person is not eligible for Polish benefits and contacts the ZUS or KRUS agency to file a claim for U.S. benefits, ZUS or KRUS sends the following information to the FSP in Warsaw, Poland:

  • A liaison form PL/USA3 certifying the filing date for the claim

  • Polish certified coverage record (PL/USA 6).

  • Available medical evidence if the claim is for disability benefits

B. Procedure – FSP process for handling claimants entitled and not entitled to U.S. benefits

This procedure explains how the FSP in Warsaw, Poland should process referrals from ZUS or KRUS.

1. Claimant Entitled to U.S. Benefits

Forward all material to OIO annotated “already entitled to U.S.” benefits.

2. Claimant Not Entitled to US Benefits

Process the claim in the same manner shown in GN 01752.010B.
