Prepare Form SSA e2960 USA/UY 3 (U.S. - Uruguay Agreement on Social Security Transmittal/Request/Certification) and forward to the Uruguayan liaison agency if the Uruguayan certified coverage record is not in the claims file.

Prepare a claims package that includes:

  Form SSA e2960 USA/UY 3 to request a Uruguayan certified coverage record if needed, to certify the filing date for Uruguayan benefits, establish factors of entitlement, and U.S. benefits information;

  U.S. totalization earnings record;

• •

  Medical evidence from SSA’s files, if the claim is for disability benefits; and,

• •

  Evidence submitted for non-disability claim.
