DI 11010.625 Field Office (FO) — Reporting Changes to the Disability Determination Services (DDS)

Procedure

Notify the Disability Determination Services (DDS) if the field office (FO) receives changes, e.g., change of address, new medical sources or evidence, report of death, withdrawal of claim, or if the courts declare the claimant legally incompetent.

Update the Modernized Claims System (MCS) or the Modernized Supplemental Security Income Claims System (MSSICS). Send any pertinent material to the DDS. For electronic cases, fax the material into the electronic folder. For paper cases, send the material to the DDS via SSA-408 (route slip). The FO can notify the DDS by using the Update-After-Transfer (UAT) Utility, see DI 81010.095. Also, the FO can inform the DDS of any changes by telephone.
