The history should include, as appropriate, the description of:
Poor weight gain;
Poor growth; and
Feeding problems (in infants).
The physical exam should include:
Blood pressure (upper and lower extremities, standing and sitting);
Heart sounds, murmurs;
Cyanosis (perioral, peripheral);
Perspiring with crying (in infants).
Laboratory testing should include, as appropriate:
Chest x-rays; and,
