As a general rule, DDSs should review:

  a minimum of the first five CE reports from all new CE providers;

• •

  a sample of reports from all providers on an ongoing basis (this can be covered by the Quality Analysis (QA) sample);

• •

  reports from CE providers that are known through experience to have a high likelihood of being deficient.

  Develop specific instructions for review of the types of CE reports outlined in DI 39545.450A.

• •

  Provide feedback to the CE provider whose report was found deficient.

• •

  Provide ways to correct the problem.

• •

  Ensure review for a reasonable period until the problem is corrected, or until the DDS is sure proper notice has been given. If the problem is not corrected, the CE provider should be removed from the panel per State procedures.
