DI 43520.025 SSA-831-U5 (Disability Determination and Transmittal) - General

The SSA-831-U5 is used to transmit disability claims to the foreign disability examiners (FDE) who act as the DDS for claims in which the disabled individual resides in a foreign country. The FDE also uses the SSA-831-U5 to record the disability determination.

The district office, CA in the MOD, or CDA in the Claims Development Branch (depending upon DO jurisdiction) is responsible for completing the identifying information in items 1-14. The CDA SGA specialist will prepare the entire SSA-831-U5 for SGA denials as outlined in DI 43520.040.
