Disability reports print one case level at a time from the “Print Forms” screen.

The following Disability Reports are available from the “Print Forms” page in EDCS:

  SSA- 3368 Disability Report – Adult

• •

  SSA-3820 Disability Report – Child

• •

  SSA-3441 Disability Report – Appeal

• •

  SSA-3367 Disability Report – Field Office

Print and secure the appropriate signatures on SSA-827s per Completing the Authorization Form SSA-827 - DI 11005.055 for instructions if necessary. EDCS defaults to print one SSA-827. You can over key the default number if more forms are needed.

EDCS can also print an SSA-831 to document SGA or technical denials (see Processing Field Office (FO) Determinations - DI 81010.140). If an FO determination is made in EDCS:

  The print screen will enable the 831 check box; and

• •

  Provide a list of claims in that case that have FO Determinations; and

• •

  The SSA-831 information generated from EDCS by the FO Determination function will be available to print.

EDCS can also print form SSA-3369 if it was completed in EDCS. The print function is available from the “Print Forms” page in EDCS.
