These questions collect information necessary to make a living arrangement determination when the claimant lives in a noninstitutional care situation. An example of noninstitutional care is when a public or private agency places a claimant under a specific program of protective placement such as foster or family care.
B. How to complete questions 23 and 24
Check the block that describes the noninstitutional care situation in question 23. Questions 24(a) and (b) collect identifying information about the facility and placement agency. The purpose of (c) is to determine the value of any in-kind support and maintenance (ISM) the claimant may be receiving.
