HI 00901.105 Attainment Cases - Card Issuance

A. Definition

Attainment cases are those in which a beneficiary was on the MBR prior to entitlement to HI benefits.

B. Process

A HI record showing entitlement to Part A and Part B benefits is automatically established for all individuals who are on the MBR and,

  • have been entitled to disability benefits for 21 months, or,

  • reach the 3rd month prior to their attainment of age 65.

At this time, a HI card is mailed showing an effective date for Part A and Part B entitlement. Instructions tell the beneficiary who does not want Part B to:

  • check the appropriate box,

  • sign where indicated, and,

  • return the card to SSA before the effective date shown on the card.

When SSA receives a card from a beneficiary who does not want Part B, Part B entitlement is removed from the MBR and HI record. A new card showing Part A entitlement only is then issued to the beneficiary.
