2. Cost of Copy:

The FO should visit the Office of Vital Statistics webpage for cost and additional information for birth, death and marriage documents.

3. Type of Payment:

 

Money order payable to Treasurer of Guam for requests for birth, death, and marriage records. Money order, cashier's check, personal check, and business check payable to the Superior Court of Guam for divorce documents.

There is a $50.00 nonsufficient funds (NSF) fee for any overdraft checks. They do accept payment via credit card, but there is a 3.1% convenience fee. Please reference the case number or invoice number when sending payment via mail.

Note: 

For an off-island request from a SSA field office, a check issued by SSA is acceptable.

 

4. Remarks:

Can FO verify, certify or obtain free copy of birth/death/marriage record? No.
