To prepare claim packages to request Swedish coverage and medical evidence and to inform SLAFU about claims for Swedish benefits, the following guidelines should be followed.
Complete Form SSA-2960-U3-SW. Check item IV.B to request a Swedish coverage record. Check item IV.D to request medical evidence from SLAFU files if claimant has filed for U.S. disability benefits.
Include the following information in claim packages.

U.S. earnings record.
Medical evidence from OIO files if the Swedish claim is for disability benefits.
