In a Disability Insurance Benefit (DIB) or Disability Freeze “open period” or “closed period” allowance, always complete Item 28.

A. Block A (Period Of Disability)

Check this block in a DIB or Disability Freeze case.

B. Block C (Estab. Beg.)

Check this block in a DIB or Disability Freeze case when the claimant is found disabled on or before the date that the earnings requirement is last met.

NOTE: Make no further entries in Item 28.
