DI 81020.210 Scanning the SSA-454-BK (Continuing Disability Review Report) into the CEF

The Disability Determination Services (DDS) is responsible for scanning the SSA-454-BK into the Certified Electronic Folder (CEF) and has the option of local scanning or contractor scanning. To scan the SSA-454 into the CEF:

  1. 1. 

    Print a barcode coversheet for Form SSA-454-BK;

  2. 2. 

    Scan and upload the SSA-454-BK to the CEF; or

  3. 3. 

    Send the paper document to the scanning contractor for imaging and retention;

  4. 4. 

    Retain the paper document according to paper retention guidelines in Document Retention and Destruction - DI 81020.055.

NOTE: The Field Office (FO) is responsible for keying the SSA-3368-BK, Disability Report – Adult, for Age 18 Redeterminations, into the Electronic Disability Collect System (EDCS). If the DDS receives a paper SSA-3368 the DDS scans the SSA-3368 into the CEF following the procedures for scanning the SSA-454-BK.
