RM 10205.085 Using Responses on Form SS-5 to Determine if the Applicant Needs an Original or a
Replacement SSN Card
Before entering information from a paper Form SS-5 (Application for a Social Security
Card) into SSNAP, you must determine the type of SSN card needed (original or replacement)
in order to know whether evidence requirements are met for the application.
These instructions apply only when the applicant is not in the office for an in-person
interview.
Follow the steps below to determine whether the applicant needs an original SSN or
a replacement SSN card, and to ensure the paper Form SS-5 is correctly completed.

Verify the SSN by requesting a Numident query or keying the SS-5 into SSNAP.

If the SSN verifies, treat as an application for a replacement SSN card.

If the SSN does not verify, request an alpha query. If the correct SSN is located, treat as an application for
a replacement SSN card. Enter the correct SSN into SSNAP and indicate “SSN - FO write-in”
in Remarks on the Summary screen.

If no SSN is located using an alpha query, treat as an application for an original
SSN. Do not enter the SSN in item 2 into SSNAP. Refer to a CR for possible earnings
discrepancy investigation if any earnings are posted.
