DI 11010.500 Claimant Prefers to Personally Request Evidence

When a claimant prefers to deal personally with a medical source, have the claimant complete an SSA-827 authorization form. Also, complete a medical report form, as appropriate, for the medical source.

Give the claimant a stamped preaddressed envelope for the DDS. In this way the medical source can mail the medical report directly to the DDS. Tell the claimant that in these cases, SSA will pay a reasonable fee for existing medical records from a non-Federal source, if a fee is charged for these records, but not for examinations or tests on which the evidence is based. This applies to both title II and title XVI cases.
