DI 11010.505 Physician Prefers to be Contacted by the Claimant

When a physician prefers to be contacted by a claimant, have the claimant complete and sign an SSA-827, authorization form. Also, complete any other medical forms as necessary. Give the claimant a stamped preaddressed envelope so that the physician can mail the medical report directly to the DDS. If the physician charges a fee for furnishing the report, the claimant should inform his/her physician that SSA will pay a reasonable fee for the existing medical evidence of record, but not for the examination or test on which the report is based.
