DI 12010.010 Assembly and Routing of Folders for Administrative Law Judge (ALJ) Hearing

A. Policy for assembly and routing folders for an ALJ hearing

The field office assembles all required forms and evidence in the proper folder order and transmits the claim folder(s) on a priority basis to the hearing office (HO).

B. Folder assembly

File the request for hearing and related materials in the proper folder order. Refer to DI 81005.050, DI 81010.090, DI 81010.125, DI 81010.135, DI 81010.150, and DI 81020.035, for electronic folders. Refer to DI 70005.001 - DI 70005.005 and DI 70010.001 for instructions on the modular disability folder procedures.

NOTE: An appeal taken on a claim filed in a two-part folder will be kept in the two-part folder.

C. Routing

For electronic cases, refer to DI 81010.085, (Transferring Cases in the Electronic Disability Collect System (EDCS)). For paper cases use a SSA 636-U3 (Transmittal Notice – Hearing Case) to transmit the claim folder(s) to the HO. Refer to GN 03103.110 for instructions on the use and completion of the SSA 636-U3.
