Use the form SSA-L8155-U2, Notice of Planned Action, to inform the recipient of any
                  adverse changes. In addition to the more usual types of adverse actions (e.g., suspension
                  of benefits, benefit reduction, termination of benefits), be sure to cover the points
                  below as appropriate:

                           SSI payments are suspended for failure to confirm eligibility (N05).

                        • 
                           The child's SSI benefits are being reduced because they no longer live in a State
                              which pays a supplement to SSI recipients.

                        • 
                           The child's eligibility for Medicaid may be affected because they no longer live in
                              a State which provides coverage. For further information, contact the Department of
                              Social Services/Public Welfare in the State of last residence.

                        • 
                           The child is not a United States citizen and, therefore, not eligible to receive SSI
                              while overseas.

                        • 
                           The child's SSI benefits are being stopped because they are no longer living with
                              a parent who is a member of the armed forces, and the child is still outside the United
                              States.

                        • 
                           The child's SSI benefits are being stopped because the parent is no longer serving
                              on permanent duty ashore overseas in the armed forces, and the child is still outside
                              the United States.

                           The child's SSI benefits are being stopped because they are no longer a “child.”
