A. EXHIBIT LETTER

We understand that you plan to visit (1) to collect your Social Security benefits.

 

What You Need to Do

You will need to give us certain information so that we can check to see if you are still eligible for these benefits. We have asked the Foreign Service post in (2) to help you fill out the forms we need. You should contact the Foreign Service post as soon as possible. This will help them serve you more quickly when you arrive at the post.

If you are eligible for the Social Security checks, we will send the checks to the Foreign Service post in (2) . The Foreign Service post will let you know when your checks have arrived.

You must pick the checks up in person at the Foreign Service post. You will need to take enough information and evidence with you to prove who you are.

B. REQUESTING INSTRUCTIONS

  • Use this notice when a beneficiary who lives in a restricted country (i.e., a country to which U.S. Government checks, including Social Security checks, may not be sent) plans to leave that country in order to collect his/ her accrued benefits.

     

Fill-ins:

    name of foreign country

  2. (2) 

    name of the city where the foreign post is located

C. TYPING INSTRUCTIONS

Use Form SSA-L2000-C2 (Universal Notice) and follow the notice standards. Information for this letter will be shown on Form SSA-573.
