RM 01301.520 How to Process Forms SSA-7014A Received From WBDOC Under Cover of Form SSA-984 With Box B.1. Checked

A. Introduction

Forms SSA-7014A that WBDOC forwards to the FO under cover of a Form SSA-984 with box B.1. checked involve individuals who have received a request from our office for corrected SSN or identifying information, and the information provided does not match SSA records.

B. Procedure

Use the following steps to process these requests for assistance:

Contact the requester and explain that the request has not been processed because the SSN, name, date of birth or sex he/she provided in response to our request for corrected information does not match SSA records.

Verify the SSN alleged by the requester.

Determine if the SSN provided by the requester matches our records:

  • If yes, obtain and develop an SSN to issue a duplicate SSN card if necessary, update or correct our records to include the name, date of birth and/or sex provided by the requester, give the requester a blank Form SSA-7004-SM-OP1, and go to Step 3.

  • If no, using the name, date of birth and SSN provided by the requester, screen for SSN, and go to Step 4.

Tell the requester:

  • that we have updated our records to reflect the information he/she provided,

  • after the duplicate SSN card is received, to complete the SSA-7004-SM-OP1 using the SSN, name, date of birth and sex used to update or correct our records,

  • to send the completed Form SSA-7004-SM-OP1 to the address on the envelope provided, and

  • that we will process the form and send him/her the requested Social Security Statement within 3 to 6 weeks.

  • STOP.

If a prior number is located:

  • verify that the SSN belongs to the requester,

  • update or correct our records (if necessary) to include the name, date of birth and sex provided by the requester,

  • use appropriate procedures to inform the SSN holder what the correct SSN is,

  • obtain and develop an SS-5 to issue a duplicate SSN card if necessary, and

  • go to Step 5.

If the incorrect SSN was used in employment:

  • initiate an earnings discrepancy investigation,

  • give the requester a blank Form SSA-7004-SM-OP1, and

  • tell the requester he/she may request a Social Security Statement after he/she has been notified that the earnings discrepancy has been resolved.

  • STOP.

If the incorrect SSN was not used in employment and a prior SSN was located:

  • give the requester a blank Form SSA-7004-SM-OP1, and

  • go to Step 4.

If the incorrect SSN was not used in employment and an SSN was assigned, tell the requester that since no SSN was ever issued or used in employment, there would be no earnings credited to our records.

  • STOP
