GN 01720.105 U.K. Coverage Certifications

A. Policy

U.K. coverage can be certified to us in either of two formats: the traditional manually prepared form UK/USA 3 or a computer generated form which combines the UK/USA 2 liaison form and the UK/USA 3. The coverage information is found in Part 9, Insurance Details, of the computer form or in Part 3, Worker's UK Insurance Record, of the manual form.

Both formats are acceptable certifications of U.K. coverage, as long as they are furnished directly by the U.K. agency to OIO or the FSP.

The UK/USA3 form should be mailed to the addresses below:

B. Description of the UK Coverage Certifications

The following information is applicable to both formats:

  • “From-to” periods are expressed in day/month/year format.

  • The periods are continuous and may overlap many years.

  • The “from” date will never be earlier than January 1, 1937.

  • Coverage prior to July 5, 1948 will always be shown in one continuous entry beginning with the later of the first date of coverage or January 1, 1937 and ending with July 4, 1948 (or earlier).

  • “From-to” periods can never be outside the person's “working life” . The working life generally runs from age 16 to age 60 for women or age 65 for men or to the year of death or the year of disability onset.

  • The “number of weeks of insurance” represents the number of weeks within the respective “from-to” period.
