GN 01732.011 How the Finnish Agency Develops Applications - Process

The Finnish Agency develops applications as follows:

When...   th en...

a claimant contacts a local Finnish agency office to apply for U.S. benefits and does not insist that the SII assist them in completing U.S. applications

 
  • the local office sends that information to the Social Insurance Institute (SII) in Helsinki.

  • the SII completes the liaison form SF/USA-2, certifies any personal information, and sends the completed SF/USA 2 to the CPSI.

  • the CPSI adds the coverage record and sends all material to the RFSP.

a claimant contacts a local Finnish office to apply for U.S. benefits and insists that the SII assist them in completing the U.S. applications

 
  • the local office sends that information to the SII in Helsinki.

  • the SII assists the claimant in completing the appropriate applications (a small supply was provided by OIO), completes the liaison form SF/USA 2, certifies any personal information, and sends the completed applications and SF/USA 2 to the CPSI.

  • the CPSI adds the coverage record and sends all material to the RFSP.

a Finnish agency needs a U.S. coverage record in connection with a claim for Finnish benefits.

    the Finnish agency requests it directly from OIO using form SF/USA 2.
