To prepare claim packages to request Greek coverage and medical evidence and to inform the Greek agencies about claims for Greek benefits, the following guidelines should be followed.
Complete Form SSA-e2960-USA-GR3. Check item V.A to request a Greek coverage record. Check item V.B to request medical evidence from the Greek agency’s files if claimant has filed for U.S. disability benefits.
Include the following information in claim packages:

U.S. earnings record,
medical evidence from DIO files if the Greek claim is for disability benefits, and
any additional evidence the claimant wants to submit including insurance workbooks (see GN 01737.212).
