When a decision is reached, advise the individual in writing of any correction or
amendment made in response to his or her request. See GN 03345.015 if all or any part of change requested cannot be made.
NOTE: If a correction or amendment to a record is made which has a bearing on a disability
determination or benefit decision, it may be necessary to consider review of the determination
or decision in regard to the correction or amendment made.
File in the claims folder or other proper file, the subject individual's:
Written request for correction or amendment,
Proof of identity, and
Proof, if any, submitted to support his or her request.
Send a copy of the corrected record and a letter indicating that corrections have
been made at the subject individual's request under the PA to any other agency or
person previously given a copy of the incorrect information.
