SI 00604.017 Other Names and SSNs and Identifying Information - Question 1(b) - (d)

A. Introduction

This information is needed for NUMIDENT verification of the SSN.

B. Procedure

1. When/How To Complete

Complete 1(b) for all claimants. If answered "YES," complete (c) and list any other names and SSNs used by the claimant. Complete (d) unless the claimant is also filing for Title II benefits. SI 00601.062 has procedures for verifying claimant identity.

2. Multiple SSNs

When reviewing the numident query, check for multiple SSNs. If the claimant has more than one SSN, also obtain Master Beneficiary Record (MBR) and SSR queries under the other SSNs. Retain the queries in the file.
