By law, every Title II and Title XVI notice issued by a field office must include the address and telephone number of the local servicing office.

When the office issuing the notice is not the servicing office, you must include a phone number and the address of the servicing office in the referral paragraph or in another section of the notice. You may show the national toll-free 800 number 1-800-772-1213 instead of the servicing office telephone number.
