GN 01708.205 Preparing Claims Packages in DIO for Claims Filed Outside Germany

A. Introduction

Initial claim packages are prepared to request German coverage records and to inform the German agencies about claims for German benefits. (See GN 01708.205 for the applications and evidence required for the packages.)

B. Procedure

1. Potential entitlement to U.S. Totalization benefits

If there is potential entitlement to U.S. totalization benefits, prepare an SSA-2960-U3-GE to request the German coverage record.

2. Claim for German benefits

If there is a claim for German benefits, prepare a claims package including:

  • Form SSA-2960-U3-GE,

  • U.S. totalization earnings record,

  • Evidence submitted for the German claim,

  • Medical evidence from anywhere in SSA's files, if the German claim is for disability benefits (See GN 01708.310),

  • Application form D/USA 1, (Application for Insurance Benefits from the German Pension Insurance) or form D/USA 2, (Application for Survivor's Benefits from the German Pension Insurance) affixed with the DIO stamp and annotated with the protective filing date in the “For Office Use Only” block on the last page or form D/USA ZA (Application for Retroactive Payment of Contributions, Insurance Benefits or Survivors' Benefits) affixed with the DIO stamp and annotated with the protective filing date in the blank space on the last page.
